The emergency information scheme,that could save YOUR life.

Follow these instructions on how to use the bottles:

• Complete the form and place it into the bottle.

• Place the bottle in the fridge

• Place one green sticker on the inside of the front door

• Place the other one on the outside of the refrigerator

• All emergency services are aware of the scheme and will use the information in an emergency

BOTTLES ARE AVAILABLE FREE OF CHARGE

From Bridgend Lions Club, Doctors Surgeries, Chemists, Neighbourhood Watch Coordinators, Social Services and many other places.
